Investigation of the Value of Test of Quantitative Fructose Semen in Diagnosis of Obstructive Azoospermia

Abstract

Background: Azoospermia accounts for 20% of male infertility, including 2 groups: obstructive and non-obstructive azoospermia. In Vietnam, epididymal puncture, an invasive method, is still used to diagnose azoospermia. The aim of the study was to survey the fructose concentration in semen and determine the correlation between the concentration of fructose and indicators of semen analysis and reproductive hormones. Materials and Methods: A cross-sectional descriptive study examined semen samples from 90 azoospermia-diagnosed male patients. Fructose level was determined according to the ROE method; volume and pH of semen, and serum FSH, LH, testosterone were also analyzed. Results: The fructose concentration in semen from the obstructive group (0.84 ± 0.94 g/l) was significantly lower than the non-obstructive group (1.56 ± 0.90 g/l), p<0.001. Fructose level was proportionally correlated to LH, FSH level, pH and volume of semen; no significant correlation was found between testosterone level and fructose level in semen. Conclusions: Lower fructose concentration was found in the obstructive azoospermia group. In addition, semen volume, pH and hormone levels were decreased in the obstructive group compared to the non-obstructive azoospermia group. A non-invasive method (semen analysis, reproductive hormones, and semen fructose concentration) can be used to determine the cause of azoospermia.
