SURFACE ELECTROCARDIOGRAM FEATURES OF LEFT FREE WALL LATERAL ACCESSORY PATHWAY DIAGNOSING IN TYPICAL WOLFF-PARKINSON-WHITE SYNDROME PATIENTS

Abstract

In 43 Wolff-Parkinson-White syndrome patients with typical left-free wall accessory pathways at Vietnam Heart Institute-Bachmai Hospital. The surface ECG parameters were compared with the localization of accessory pathways identified as described above. Results: PR interval in the left group was 108.37 + 10.9 ms and QRS duration was 127.12 + 10.76 ms in the left group. Positive delta wave in V1 found in 97.67% of the left group. The positive delta waves in at least 2/3 inferior leads of the common in the including both anterolateral and left lateral accessory pathways of the atrioventricular vales occurred in 92.11% of cases, while the negative delta waves in at least 2/3 inferior leads of the common in the left posterolateral accessory pathway of the atrioventricular valve accounted for 100,0% of cases. The R/S ratio > 1 in V1 of the anterolateral accessory pathways and R/S ratio < 1 or R wave in V1 of the left lateral accessory
pathways. Conclusion: The surface ECG parameters in typical WPW syndrome closely related to localization of accessory pathways during cardiac electrophysiological testing and can be used to predict the localization of accessory pathways.
