BEDSIDE 20 MINUTE WHOLE BLOOD CLOTTING TEST IN DIAGNOSIS OF VIPER SNAKEBITE

Abstract
The aim of this study was to evaluate the diagnostic value of 20
minute whole blood clotting test (20WBCT) in victims of viper snakebite. A retrospective
observational study included of 43 patients with viper snakebite and 74 patients with
cobra snakebite treated in Poison control center of Bach Mai Hospital from May 2011 to
August 2012. Results showed that among 43 patients with viper snakebite, 20 patients
had 20WBCT positive, all 74 cobra snakebite patients had 20WBCT negative. 20WBCT
positive only in patients with viper bite after 6 hours. 15 cases present hemorrhagic
clinically and all were positive with 20WBCT. 28 patients with no signs of clinical
hemorrhage (65.2%), however 5 patients in this group had 20WBCT positive (17.9%).
20WBCT reflecting very good coagulopathy condition causes bleeding (100%) and
valuable early detection coagulopathy disordors without clinical manifestations. In
summary, bedside 20 minute whole blood clotting test showed early diagnosis value for
coalgulopathy even when there was no clinical symptoms and thus provides early
indication for antivenom therapy in patients with viper snakebite.
