CHARACTERISTICS OF MULTIPLE MYELOMA PATIENTS WITH RENAL FAILURE IN HEMATOLOGY AND BLOOD TRANSFUSION CENTER AT BACH MAI HOSPITAL FROM 2016 TO 2019

Abstract
Renal failure is seen in a high proportion in patients with multiple myeloma, which is also a poor prognostic factor. Objectives: 1) Describe the characteristics of renal failure in multiple myeloma patients with renal failure. 2) Analysis of the relationship between kidney failure and disease characteristics. Methods: 318 newly diagnosed multiple myeloma patients following the IMWG 2014 standard, including 83 patients with renal failure. Patients with renal insufficiency were tested for renal function. Results: The rate of renal failure was 26.1% (83/318), of which 16.9% (14/83) had eGFR 30-59 ml / min / 1.73m2 skin, 51.8% (43/83) had eGFR 15-29, 31.3% (26/83) had eGFR <15. Kidney size: 92.8% normal, 6% atrophy, 1.2% to. Kidney morphology: 28.9% normal, 21.7% negative boundary demarcation, 36.1% clear demarcation, 13.3% have chronic kidney disease image. eGFR <15 had a high incidence in lambda light chain (53.5%), stage of ISS 3 disease (58%). The concentration of beta 2 microglobulin in eGFR groups <15, 15-30, 30-60 were 6.38, 6.78, and 19.9 respectively. The correlation coefficient between eGFR and kappa concentration is -0.288, with lambda concentration of -0.013. Conclusion: Renal failure accounts for a high proportion in patients with multiple bone marrow tumors. Disease form, stage, Beta 2 microglobulin level, mild serum chain concentration associated with the degree of renal failure.
