THE RELATIONSHIP BETWEEN THE DIFFERENCE IN MEDIAN-VERSUS-ULNAR LATENCY, IN MEDIAN-VERSUS-RADIAL LATENCY AND CLINICAL SIGNS & SYMPTOMS IN PATIENTS WITH CARPAL TUNNEL SYNDROME

Abstract

Nerve conduction study (NCS) is a valid and reliable mean of evaluating the conduction speed of medial nerves in patients who have carpal tunnel syndrome (CTS). Objectives: (1) To assess the NCS parameters (DSLD IV, DSLD I, DMLd, DSLd) of medial nerve. (2) To analysis the the correlation between these NCS parameters with clinical signs & symptoms in CTS patients. Methods: This is a descriptive, cross-sectional study, was carried out on 103 hands of 60 patients with clinical signs and symptoms that suggested CTS, randomly selected at the medical center No.1 Ton That Tung, Hanoi Medical University. These patients were then examined about clinical signs & symptoms and the nerve conduction study of medial nerves. Results and conclusions: Distal motor and sensory latency differences between medial nerves and ulnar & radial nerves were DMLd = 2,31 ± 1,63 m/s; DSLd = 1,05 ± 0,61 m/s; DSLd IV = 1,37 ± 1,03 m/s; DSLd I = 1,02 ± 0,63 m/s with the highest abnormal frequency were 82,52%, 80,58%, 91,26%, 83,49%, respectively. There was no correlation between common clinical characteristics (female (91,7%), middle-aged (40-60 tuổi) (81,6 %), dominant hand (57,3%)) and four NCS parameters mentioned above (p > 0,05), where as sensory deficit (r = 0,732), thenar eminence atrophy (r = 0,692), muscle weekness (r = 0,676) and positive Durkan test (r = 0,662) had a strong positive correlation with DMLd index. In both cases the correlation was statistically significant (p value < 0,01).
