HOMOCYSTEINE LEVELS, CAROTID ARTERY STENOSIS AND ASSOCIATED FACTORS IN PATIENTS WITH TYPE 2 DIABETES MELLITUS

Abstract

Objective: To evaluate homocysteine levels, the carotid artery stenosis severity, and associated factors in patients with type 2 diabetes mellitus. Materials and Methods: A cross-sectional study was conducted on 113 patients with type 2 diabetes mellitus aged 18 years and older at Can Tho University of Medicine and Pharmacy hospital. Results: Carotid artery stenosis was detected in 40.7% of patients, with mild stenosis being the most common severity (30.1%). Patients aged ≥ 60 years and those with a diabetes duration of ≥ 10 years had a significantly higher prevalence of carotid artery stenosis (p= 0.02 and p= 0.004, respectively). Elevated homocysteine levels were observed in 18.6% of patients, with a mean plasma homocysteine concentration of 8.88 ± 4.50 μmol/L. Patients aged ≥ 60 years, those with a diabetes duration of ≥10 years, and those with fasting plasma glucose ≥ 7.2 mmol/L had significantly higher rates of hyperhomocysteinemia (p= 0.015, p< 0.001, and p= 0.013, respectively). Patients with elevated homocysteine had a 3.75-fold higher risk of carotid artery stenosis than those with normal homocysteine levels (95% CI: 1,38-10,23; p= 0.007). Conclusion: Carotid artery stenosis was present in 40.7% of patients, while 18.6% had hyperhomocysteinemia. Patients aged ≥60 years and those with a diabetes duration of ≥ 10 years had a higher prevalence of carotid artery stenosis. Elevated homocysteine levels were more common among patients aged ≥ 60 years, those with a diabetes duration of ≥10 years, and those with fasting plasma glucose ≥ 7.2 mmol/L.
