BLOOD GAS CHARACTERISTICS AND SERUM OSMOLARITY CHANGES IN PATIENTS WITH METHANOL POISONING TREATED AT POISON CONTROL CENTER OF BACH MAI HOSPITAL

Abstract

The study series of 30 patients treated at the Poison Control Center
of Bach Mai hospital from 01/2010 to 06/2014 aimed to assess blood gas characteristics,
anion gap and osmol gap in patients with methanol poisoning. The study results showed
that methanol poisoning mainly in men (80%), age group of 40- 59 years old (63.3%),
56.6% had a history of alcohol abuse. Among study population, 50% had metabolic
acidosis with pH of 7.11, interquartile 6.77 to 7.22; low level of HCO3 (median 5 mmol/l),
median of pCO2 was 24 mmHg, BE was -20 and lactate was 7.1 mmol/l. After treatment,
pH, HCO3, BE and blood lactate of all patients were normalized. 22 patients present with
elevated anion gap (median 31.4) and 100% increased osmol gap (median 58 mosmol/L).
Methanol concentrations were above threshold of ocular toxicity in 14 patients and 10
patients present with fatal level (above 50 mg/dL. Blood methanol concentrations
correlated with anion gap with r=0.575. In summary, metabolic acidosis with an elevated
anion gap is a hallmark of methanol poisoning, charges of anion gap, and osmol gap were


valuable for diagnosis and management of methanol poisoning.
