THE DIAGNOSTIC VALUE OF MEDIAN NERVE CONDUCTION STUDIES IN CARPAL TUNNEL SYNDROME

Abstract

Objectives: 1) Describe the clinical characteristics and the indexes of motor and sensory
median conduction in patients with CTS; 2) Identify the sensitivity and specificity of the indexes of motor and sensory median conduction in diagnosis of CTS. Methods: descriptive study. 48
patients with a diagnosis of CTS were included in the study. All patients underwent clinical
interview, physical examination and electrodiagnostic examination to determine clinical and electrophysiologic features of CTS. Results: Among 48 patients with CTS, hand numbness was the most common (89.3%), other common symptoms were numbness when driving (68.0%), numbness at night (58.7%). Mobility symptoms often appear in the later stages of the disease. Sensitivity of Phalen's test, Durkan's test and Tinel signs were 72.0%, 57.3% and 36.0%,
respectively; specificity were 62.5%, 75.0% and 72.5%, respectively. The threshold for abnormal assessment of DMLm was 4.05 ms, DSLm was 2.95 ms, DMLd was 1.65 ms, MULD was 1.15 ms and MRLD was 1.70 ms. The DMLd parameter has the highest sensitivity (91.7%) but the lowest specificity (80.5%), the second highest sensitivity is DMLm (87.5%). The parameters DMLm, DSLm, MULD and MRLD have high specificity in the range of 90-100%. Conclusions: The indexes such as: DMLd, MULD, DMLm, DSLm have a great significance of diagnosis and grading CTS.
