ASSOCIATION BETWEEN LEFT VENTRICULAR EJECTION FRACTION AND NUTRITIONAL INDICES IN PATIENTS WITH CHRONIC HEART FAILURE

Abstract

Objective: To examine the association between left ventricular ejection fraction (LVEF) and composite nutritional indices, including the Prognostic Nutritional Index (PNI), Geriatric Nutritional Risk Index (GNRI), and Controlling Nutritional Status (CONUT), in patients with chronic heart failure. Methods: This prospective observational study included 733 patients with chronic heart failure treated at Can Tho Central General Hospital between January 2023 and March 2024. Results: Among 733 patients, 602 (82.1%) had HFrEF, 81 (11.1%) had HFmrEF, and 50 (6.8%) had HFpEF. Patients with reduced EF had lower albumin, PNI, and GNRI values and higher CONUT scores; BMI and waist circumference differed significantly across EF groups. In multivariable linear regression, PNI was positively associated with LVEF (β = 0.74; 95% CI: 0.71-0.77; p < 0.001), GNRI was positively associated with LVEF (β = 0.57; 95% CI: 0.53-0.60; p < 0.001), whereas CONUT was inversely associated with LVEF (β = -1.08; 95% CI: -1.41 to -0.76; p < 0.001). Conclusion: In patients with chronic heart failure, LVEF was significantly associated with composite nutritional indices. Higher PNI and GNRI were associated with better left ventricular systolic function, whereas higher CONUT scores were associated with lower LVEF.
