Abstract

Background: Single all-ceramic crown restoration in the anterior region remains a clinical challenge because it must simultaneously meet the requirements of esthetics, function, mechanical strength, and periodontal tissue compatibility. CAD/CAM-fabricated zirconia crowns have been increasingly used due to their high strength, biological stability, and favorable esthetic performance in the anterior region. Objective: To evaluate the clinical outcomes of anterior tooth restoration with CAD/CAM-fabricated zirconia crowns according to the modified United States Public Health Service (USPHS) criteria after 12 months. Materials and methods: This was a prospective, uncontrolled clinical interventional study conducted on 60 patients with 80 zirconia crown restorations in the anterior region at Can Tho University of Medicine and Pharmacy Hospital from June 2020 to September 2021. Clinical outcomes were assessed at 3, 6, and 12 months using the modified USPHS criteria, together with evaluation of gingival status and patient satisfaction. Results: Females accounted for 65.0% of the study population, patients younger than 25 years accounted for 75.0%, and 96.7% of patients sought treatment for esthetic reasons. The main indication for restoration was dental caries, accounting for 82.5%, followed by trauma at 12.5% and wedge-shaped deformity at 5.0%. Maxillary/mandibular central incisors were the most commonly restored teeth, accounting for 72.5%, and vital teeth accounted for 67.5%. The mean ratio of alveolar bone height to root length was 88.0 ± 3.5%, and alveolar bone loss was recorded in 61.3% of cases. After restoration, gingival status gradually improved, with the proportion of restorations with good gingival status increasing from 88.0% at 3 months to 100% at 12 months. The modified USPHS criteria achieved Alpha ratings in 95.0% to 100.0% of cases, with no clinical failures recorded. After 12 months, the overall outcomes were rated as good in 92.5% and fair in 7.5% of restorations; 100% of patients were satisfied with the treatment. Conclusion: CAD/CAM-fabricated zirconia crowns for anterior teeth showed favorable clinical outcomes after 12 months, meeting the requirements for esthetics, function, and gingival tissue compatibility.
