SHORT-TERM CLINICAL OUTCOMES OF CROWN-LENGTHENING SURGERY COMBINED WITH PROSTHETIC RESTORATION IN MOLARS WITH SUBGINGIVAL TOOTH STRUCTURE LOSS

Abstract
This study evaluated clinical outcomes of crown-lengthening surgery combined with prosthetic restoration in molars with subgingival tooth structure loss. The longitudinal uncontrolled analysis included 34 molars in 34 patients from a cohort established at Can Tho University of Medicine and Pharmacy Hospital during 2021-2023. The previous cohort publication reported baseline clinical and cone-beam computed tomography characteristics only; the present study reports post-treatment outcomes. Thirty-three cases were complete at the 2023 residency-thesis data lock, and one additional original-cohort case completed follow-up later; no new participants were recruited. Periodontal indices were assessed before surgery, at one week, and at six months; restorative outcomes were evaluated immediately after cementation and after two months. Females accounted for 58.8%, patients aged 18-30 years for 64.7%, mandibular molars for 88.2%, and caries for 82.4%. At six months, gingival index, plaque index, and probing depth decreased significantly. Keratinized gingiva decreased but remained 4.18 ± 0.97 mm, whereas supracrestal tissue attachment was unchanged (p=0.280). Surgical success was recorded in 32/34 cases (94.1%). All-ceramic restorations accounted for 73.5%; proper occlusion and marginal adaptation were achieved in 97.1% and 94.1%, respectively. At two months, 91.2% had good masticatory function and 88.2% reported good satisfaction. The combined treatment showed favorable short-term outcomes in appropriately selected molars; longer follow-up is required.
