HORMONE LEVELS IN PREREPRODUCTIVE FEMALE THALASSEMIA PATIENTS WITH IRON OVERLOAD AT THAI NGUYEN CENTRAL GENERAL HOSPITAL

Abstract

Background: Female Thalassemia patients in the pre-reproductive age are severely affected by iron overload, leading to endocrine dysfunction and impaired reproductive development. Objective: To determine the levels of selected female reproductive hormones and evaluate the relationship between hormone levels and iron overload. Methods: A cross-sectional, retrospective study was conducted on 45 female patients aged 12 – 18 years, treated at the Hematology Department, Thai Nguyen Central Hospital. Hormones measured included LH, FSH, Estradiol, Progesterone, and Prolactin. Iron overload was assessed using biochemical indices (serum ferritin, serum iron). Results: The mean hormone levels were as follows: • LH: 4,4 ± 2,6 mIU/mL • FSH: 6,8 ± 2,9 mIU/mL • Estradiol: 60,5 ± 31,5 pg/mL • Progesterone: 4,6 ± 13,4 ng/mL • Prolactin: 185,9 ± 136,0 mIU/mL Female reproductive hormones decreased proportionally with the degree of iron overload. Iron overload showed negative correlations with LH, FSH, Estradiol, and Prolactin, and a positive correlation with Progesterone. The prevalence of delayed puberty and hypogonadism increased with the severity of iron overload. Conclusion: Iron overload has a significant impact on female reproductive hormones in Thalassemia patients. Early monitoring and intervention are essential to prevent future reproductive dysfunction.
